When creating a form, it should be as simple as possible. Of course, you can get creative with stylizing it, but at its core, it should be easy to navigate. When it comes to those using screen readers, it may be difficult to understand which text box, radio button, checkbox, etc. their mouse is hovering on.

It is important to label each option in your form with corresponding, accurate text.
